Eligibility Individuals who choose hospice care have been diagnosed with a life-limiting illness. This illness may be a type of cancer or a disease of an organ such as the heart, lung, liver, kidney, nervous or immune system.
The clinical extent of the disease along with other factors helps the physician determine that an individual has a life expectancy of six months or less should the disease follow its normal course. Hospice is right for you when you and your physician have decided that the primary goal of treatment is no longer cure but rather comfort and symptom management. Because the team members at Faith and Grace Hospice are committed to compassionate end of life care, we ask our patients to identify a friend or family member as their designated caregiver. This caregiver oversees the care provided in the home and is often supported by other caregivers. Services Faith and Grace Hospice provides palliative care that focuses on comfort and quality of life when curing the disease is not a realistic possibility. Our goal is to meet the unique physical, emotional and spiritual needs of the patient and family as they transition through end of life. We do this with respect and honor and feel very privileged to participate in your care. Hospice care is delivered at home, where most people want to be when they are ill, but may also be provided in other healthcare settings.
The services available to you through hospice care are related to your terminal illness and are utilized to provide comfort, safety and symptom management. Each service is authorized by your personal physician and the hospice team. These professional services become part of your plan of care and include:
Faith and Grace Hospice offers an organized bereavement program available to caregivers for at least one year after a patient’s death. There is no right or wrong way to grieve after a loss. We all grieve in our own way and grief has no timetable. After a loss, you may be overwhelmed by grief reactions you are experiencing. Our organization offers counseling provided by our bereavement team. We continue to communicate with our caregivers through phone calls, bereavement visits, educational mail outs and offer a grief support group. Life won’t be the same after your loss, but experiencing your grief will allow you to adjust to life after loss. Cost and Coverage Hospice care is reimbursed through Medicare, Medicaid and most private insurance companies. When you elect hospice care, the hospice is directly reimbursed by Medicare, Medicaid or your insurance company. Medicare Medicare offers the Hospice Benefit to individuals who are eligible for Medicare A and meet the general eligibility requirements. Traditional Medicare benefits remain unchanged for the treatment of medical issues unrelated to the terminal illness. When all eligibility requirements are met, the Medicare Hospice Benefit covers the cost of your hospice care. Medicaid Like Medicare, Medicaid covers hospice services. Private Insurance Faith and Grace Hospice, Inc. will work with you to maximize any hospice benefits available under your private insurance coverage. Your care will never be determined by your ability to pay. How to Make a Referral Normally, a referral is initiated by a physician. Once the referral is received, the patient is contacted to make him/her aware of the referral and to make sure that hospice is an option they’d like to explore. General information can be shared over the phone but more specific information is discussed during a meeting in the home. All information is kept strictly confidential. Neither the phone call nor the visit obligates the patient or family to hospice care. |
