Faith and Grace Hospice is a nonprofit organization that provides care to the terminally ill patient and their family. It is an organization with the goal to serve all patients in a manner that is respectful and honors the dignity of each individual.

Faith and Grace Hospice offers a caring partnership to the patient, family and physician. Our professional services, skills and relationships shall be to honor persons and encourage peace and hope consistent with their belief system. These services are relationship centered and based upon truth of information and trust of commitment to the integrity, dignity and values of each patient and family as they transition to the end of life.

History

Faith and Grace Hospice was born out of a love for life and respect for the continuum that end of life represents. In 2005, the combined efforts of nursing, social work and clergy culminated the foundation of our professional healthcare organization. Strengthened by experience and conviction, we continue to grow each day with a goal to provide excellent patient care.

Faith and Grace Hospice promotes general well being, comfort, dignity and spiritual strength through a caring partnership with patients, families and physicians. It is by the grace of giving, a welcomed presence and the relationship we share that allows Faith and Grace to provide professional, compassionate end of life care.

The Hospice Team: Meet our Staff

Faith and Grace Hospice offers a caring partnership to the patient and family.

Your hospice team includes your personal physician, the hospice Medical Director who is also a physician, an RN Case Manger, Certified Nursing Assistant, Chaplain, Social Worker, Volunteer Coordinator and volunteers. Therapy services may be available to you for comfort and safety needs.

Your personal physician remains involved in your care throughout your hospice stay and orders your medications, services and equipment. The hospice staff becomes an assistant to your physician by providing ongoing information regarding your care and communicating any needs or concerns you may have.

The Medical Director oversees the care provided by the hospice team and acts as a resource or expert medical advisor.

While you are with hospice, you will be visited by the same nurse on a regular basis. The frequency of those visits will be determined by your needs and wishes and may vary throughout your hospice care. Your nurse will assess your general well being, review your medications and discuss any questions or concerns you or your caregiver may have.

Nursing assistants will be available to help you with personal care such as bathing and dressing, and with general activities such as changing the linen on your bed and straightening your living area. Your nursing assistant is someone else you will see on a regular basis.

Faith and Grace Hospice offers and provides spiritual care beyond the specific faith group litany and rituals. Our Chaplain serves as an additional support to you and your family and is available to visit or call as your particular needs, desires and comfort require.

Our social workers support and provide counseling for you and your family and are expert at accessing community resources / information that may be helpful to you. The social workers also follow up with your family members though support groups and bereavement care.

The volunteer Coordinator and Volunteers are always eager to help. Your volunteer might provide support for you in a variety of ways - conversation, reading, personal comfort activities, feeding, listening to music, assisting with errands, and several other helpful activities.

You are the number one hospice team member. Your participation in determining your plan of care is very important. We strive to help you meet your goals and the goals of your family / caregivers through a trusting relationship and ongoing information sharing.

All hospice team members have received training that allows them to meet your physical, psychosocial, spiritual and emotional needs. This training, experience and background establishes the foundation for a meaningful relationship. We encourage you to meet all members of our team to determine your unique need and or desire for their involvement in your care.
